Substitution/Waiver Forms
Substitution/Waiver forms are required for any deviation from published curriculum. They can be submitted while a student is enrolled in a relevant course, or any time after. Sub/Waivers are not required for graduate students with a Plan Of Study document.
Reminder: Students are attached to a specific catalog year, not the current catalog. To determine what catalog year your advisee is attached to, go to Campus Connection > Faculty & Advisor Homepage > Advisor Center > View Student Details > Other Academic Information Dropdown > Academic Requirements > Click the » button. The catalog year will show up in the list of requirements. It may be simpler for some students if we switch them to a newer catalog year (one major change form versus multiple sub/waivers).
Sub/Waiver forms are available in Campus Connection > Faculty & Advisor Homepage > eForms > Registration & Records > Admin-Request to Waive/Sub
Procedure:
- Enter the student’s ID number. The form will populate their programs.
- Enter anticipated graduate term and academic career.
- Enter the program under “Substitution/Waiver is for” a. A Sub/Waiver can only be used for ONE degree plan. Music Education Undergrads have two plans, major and minor. If a class is being substituted for both the major and the minor, that requires two sub/waivers.
- Under “COURSE WAIVER DETAILS” please describe the courses you would like to Waive (not courses you would like to substitute). a. Please be careful with the wording "Is no longer required" when discussing degree requirements. Students matriculate under a specific year's curriculum, and when we make curricular changes it is not retroactive. Students are required to take the courses listed in the curriculum, unless we have a compelling reason not to. For example, EDUC 475: Reading in the Content Area was briefly required for licensure under state law. It is no longer required for licensure, but it IS required for the Music Education degree for some older students. The wording "EDUC 475 is no longer required for licensure and is no longer offered" is appropriate in this instance.
- Under “SUBSTITUTION DETAILS” enter the original requirement and the substitution, one per line. Do not include courses we will be waiving.
- Enter any additional comments you might have.
- For Departmental Representative, Kyle and Tyler are approvers for Music. Music Ed undergraduate students require Teresa Shume (Teresa.shume@ndsu.edu) as a secondary approver, to ensure that we are not substituting/waiving any courses that would impact licensure.
